This acknowledgment should be presented before checkout for Surf Lessons and Surf Guiding. It supports clear safety communication and does not remove responsibility or consumer rights that cannot legally be excluded.

Participant acknowledgment

I understand and agree that:

1. Surfing and ocean activities involve changing natural conditions and risks, including waves, currents, falls, collisions, contact with surfboards or the seabed, marine life, weather exposure and physical injury.
2. I have provided accurate information about every participant’s age, swimming ability, surfing experience and other information relevant to safe participation.
3. I will tell SW Surf before the activity if a participant has a condition, injury, medication or limitation that could materially affect safe participation or emergency response.
4. Each participant will follow the instructor’s or guide’s directions and local beach rules.
5. No participant will take part while affected by alcohol, illegal drugs or medication that makes participation unsafe.
6. SW Surf may change the beach, time, route or activity plan, or stop participation, when reasonably necessary for safety or suitability.
7. Wave quality, weather, crowd levels and access to a requested surf spot cannot be guaranteed.
8. Personal belongings brought to the beach or vehicle remain the participant’s responsibility, subject to rights that cannot legally be excluded.

Additional acknowledgment for Surf Lessons

I understand that:

• the minimum lesson age is 10;
• a participant who cannot swim must be identified to SW Surf before confirmation;
• a standard two-hour lesson normally includes approximately 90 minutes in the water;
• a parent or authorised guardian must make the booking and provide consent for a minor.

Additional acknowledgment for Surf Guiding

I confirm that every guiding participant can swim and paddle out. I understand that:

• surf guiding is not a complete-beginner or non-swimmer service;
• the guide may change the location or recommend a lesson if the participant’s actual ability differs materially from the information provided;
• the standard guided water time is approximately 90 minutes;
• any additional surfing time, up to one hour, is independent and Mohamed will not remain in the water;
• I remain responsible for deciding whether to continue during the independent period.

I have read and accept the Safety and Participation Acknowledgment. I confirm that the swimming and surfing information I provided is accurate and agree that all participants will follow SW Surf’s safety directions.

Record the booking reference, accepted document version, date and time. For a minor, record the responsible adult’s name and confirmation of authority to consent.
